Nominations

This page provides the viewer with a clear understanding of the vital information to gather and the necessary procedures to undertake prior to submitting a nomination. Be sure your facts are accurate (including dates). The Selection Committee will research and verify the information on your application.

There are three inductee classifications into the Hall: athlete, builder, or athlete/builder. Eligibility requirements are listed for each category.

Criteria for induction into the Kingston & District Sports Hall of Fame, nomination form, deadline for nominations, and where to send your presentation is provided at this location. All the information is in one document. 

The five page nomination form must be received by  SEPTEMBER 15,  each year, regardless of which day it falls on.

Nominations cannot be completed on-line. Please download and print one of the files below.  Please note that the date on the nomination documents may still be October 15; that is incorrect.
